review the District's attendance
map, but are encouraged to call Central Services,
790-6400, to confirm which school their student
will attend.
Spanish versions of the basic registration packet
may be obtained by calling Central Services at
790-6400. Assistance in completing the forms may
be available from your school office or Central
Services.

2008-2009 Registration Download Center
Select the registration materials for the school your student(s) will attend from the list below.
Download the Basic Packet and forms you need. Print them, fill out all forms contained, then send to the District. All registration materials and forms should be returned to the District 41 Central Services Office, located at 793 N. Main Street, Glen Ellyn, IL 60137. If you would prefer to have a packet mailed to you, please contact us at 630-790-6400.
