Dear Parents/Guardians:

We are requesting your assistance in gathering data regarding pre-school age students that live within our district boundaries.  This data will also be used to create mailings to families as their children become eligible to attend our kindergarten programs in the future.

If you don’t have any pre-school age children, but know of others, please pass this letter/form on to those families or have them call the school office or the Central Services Office for more information.  Our phone numbers are listed below.

If we already have information on your child or if you have not already done so, please complete the form above and return it to your local school office or Central Services Office as soon as possible.  You can also fax the completed form to Central Services at 630-790-6540.
