Pre-School Screening Information

All pre-school students that receive services through District 41 have been developmentally screened and/or evaluated and have qualified for services.

District 41 conducts developmental screenings for children, ages 2 years 10 months through age 5 using the Ages & Stages Screening Materials.

Step 1: Contact District 41 by calling 630-534-7308 to receive an Ages & Stages Screening packet. We will send out the packet within 7 business days of your initial call. Complete all information in the packet and return it in the enclosed self-addressed stamped envelope.

Step 2: Upon receipt of your completed packet, we will review the information and you will be notified within two weeks either by email or in writing of our findings and/or the next step in the process.
